           Typing in your name below means you understand and agree with all terms and conditions of this release.

I have full and complete understanding: (1) of the physical requirements of participating in the Chapel Chase; (2) that it takes place on public streets; and (3) that medical personnel may not be available. In return for being allowed to participate in the Chapel Chase, and with an understanding of the risks of personal injury inherent, in participating in the event, I voluntarily RELEASE Mission Haven Rediemed Foundation (MHR), INC. and its founders, employees, trustees, board members, agents, volunteers, and representatives from ALL claims and lawsuits relating to any loss, illness, damage or injury, including suffering, short term injury, long term injury and death, which I may sustain while participating in (or traveling to/from) the Chapel Chase, including but not limited to claims based on negligence, recklessness, strict liability, and fiduciary duty. I represent that I am physically able to participate in the Chapel Chase. If I am injured or become ill during the Chapel Chase, I will seek medical help on my own as this is a virtual event. I again agree that Mission Haven Rediemed Foundation, INC. has no responsibility for any expense, injury, or damage that may arise out of or be in connection with authorized emergency medical treatment.
